US Salsa Open July 12th – 15th, 2012

All competitors must sign this form to be accepted as a valid entry into this event.

ALL COMPETITOR MUST REGISTER PRIOR TO EVENT PRIOR TO JUNE 30TH, 2012 AFTER THAT A $20 LATE FEE CHARGE PER EACH ENTRY WILL BE ADDED

Entry Deadline is JUNE 30TH,2012.

• · The US SALSA OPEN CHAMPIONSHIPS and participating dance companies and studios do not accept responsibility for any lost or stolen personal items left unattended in the changing rooms, ballroom, or hotel rooms.
• · The US SALSA OPEN CHAMPIONSHIPS and PB&G Productions, Inc. will not be held liable for injuries sustained by persons attending this event no matter if said attendants come as spectators, officials, competitors, or guests of the organizer. Everyone will attend and participate at his/her own risk.
• · The US SALSA OPEN CHAMPIONSHIPS and PB&G Productions, Inc. forbid all unauthorized filming and/or video taping at this event. A licensed professional video production company will be present to document the event and provide DVDs for purchase. TV production of the event will be made available for purchase and/or television viewing. (NOTE: No competition or performance is guaranteed appearance on the DVDs or television network. All will be displayed at the discretion of the producers and television network.)
• · The US SALSA OPEN CHAMPIONSHIPS is not liable for any personal charges made at the hotel. All participants must pay their own charges, including phone charges, room service, etc.
• · There will be no smoking allowed in the ballroom, hotel rooms or changing rooms.
